Medical Health Plans - 2023 Update

If your child is required to have medication administered during school hours including a ventolin inhaler, hayfever or prescribed medication, can you please fill out the attached permission administer form. An Asthma/Anaphylaxis Action Plan will need to be supplied if administering ventolin or epipen. Short term medication eg. antibiotics or pain relief can also be administered. Medication to be supplied in a bottle/packet with a pharmacy label detailing the child's name and dosage required.
